'93 Softail shocks problem

I had a pair of new Progressive 422 shocks installed on my '93 Heritage Classic and after only a few weeks and about 500 or so miles the rear bushing on the left shock is destroyed and breaking apart. What would cause this? Would it be a defect in the rubber bushing or could it be from the shocks being adjusted unevenly by the mechanic either on the preload or the ride height adjustment?

The shop that did the work will contact Progressive on Monday as will I. This shock replacement project is an ongoing 3 month effort
to get the job done right and in such cases I find that it's best to know the answers before you ask the concerned parties the questions.
