cam plate plug backed out

Working on my budys kids 04 road king doing a gear drive conversion. The plug for the oil galley at the top of the cam plate backed out. The only thing that stopped it from coming out completely was that it hit the cam cover. The o-ring is intact and looks to be in good shape. The pn. in my 99 dyna book is 3290 and I would think they are the same for the 04. The bike has 29k. on it and we inspected the tentioners about 10k. ago and replaced one. Everything was fine at that time. Has anyone seen this problem before?
