Cycle Shack Part Number PHD323. Looks just like this. Works good for me, not sure what the cost is, as it was on the bike when I got it. Looking at their website, it looks like you can use mid-controls, even though mine has forwards.
I ran a Thunderheader for years-Worked great on 80 " Evo's and any motor for me up to 107"-Then I went with a JBV Racing Pipe for my big engines.
