2005 XB12R Crankcase Berings

My crankcase berings failed at 22k damaged piston/cyls. I'm wanting to understand the different reasons for this so early on.
Why may this have happened? How can it be prevented?

59660 is on a distinguished road
First, was it the left side timkens or the right side rollers, or was it the con rod bearings?
Second, what were the conditions at failure..ambient temperature, engine rpm, type and weight of oil?
